This assessment is the first step into Calyx Health - a comprehensive evaluation designed to look beyond isolated symptoms and understand the bigger picture.

Before your appointment, you'll complete a detailed health questionnaire covering your history, symptoms, medications, supplements, lifestyle, and goals.

A focused laboratory evaluation looking at key metabolic, cardiovascular, thyroid, nutritional, and general health markers.
💻 60-Minute Personalized Consultation
We'll review your health story, discuss your goals, and interpret your laboratory findings in the context of you, not just the reference range.
🌿 Personalized Foundation Roadmap
You'll receive a written roadmap outlining your key priorities and your initial recommendations.
🥑 Nutrition & Lifestyle Recommendations
Practical recommendations based on your goals and current health picture.
💊 Initial Supplement Recommendations
When appropriate, you'll receive 1–3 targeted supplement recommendations based on your individual needs.

Are labs included?
Yes. Your foundational laboratory evaluation is included in the Foundation Assessment investment.
Is medication included?
The Foundation Assessment is primarily an evaluation and foundational planning experience. If medication management is clinically appropriate, we'll discuss that as part of your recommended next phase of care.
Will I receive a treatment plan?
You'll receive personalized nutrition, lifestyle, and initial supplement recommendations along with your Foundation Roadmap. More comprehensive treatment and medication management are available through our ongoing Calyx programs when appropriate.
Do I need to know which program I need?
No. That's one of the reasons the Foundation Assessment exists. We'll determine the most appropriate next step based on your health history, goals, symptoms, and laboratory findings.
How long is the appointment?
60 minutes.
Is this covered by insurance?
Calyx Health is a cash-pay practice. [Then whatever exact wording you want regarding labs/insurance based on your actual setup.]
